My Story

Jacqueline Chen President of JCAN, has served on the JCAN board for 5 years. Jacqueline has over 25 years of professional nursing and health leadership experience. She has administrative leadership expertise in the areas of primary care/quality, clinic operations, and start-up, strategic planning, and health equity-informed decision-making.

She is a Clinical Leader Manager at the St. Michael’s Hospital Academic Family Health Team. Jacqueline earned her BSN from Ryerson University and her MSN at Athabasca University. She holds the Certified Health Leaders Designation (CHE) from The College of Health Leaders. Jacqueline has advanced training and certification in senior-level change management from the Rotman School of Management, University of Toronto, and certification in quality improvement and patient safety, University of Toronto.

Jacqueline is a Certified Diabetes Educator with over 20 years of clinical expertise. Jacqueline serves on the Board of the Inner City Family Health Team, Toronto, Ontario.
